Terms and Conditions

1. Scope of Services

Dr. Wacks provides professional medical coaching and telemedicine services specializing in the evaluation and management of peri-menopause and menopause. Services may include, but are not limited to:

Comprehensive health and hormone assessments
Lifestyle, nutrition, and wellness counseling
Evidence-based treatment recommendations within the scope of gynecology and functional medicine
Ongoing follow-up care through telemedicine

All care is individualized and delivered in accordance with the American College of Obstetricians and Gynecologists (ACOG) guidelines, protocols, and standards of care.

2. Licensing and Jurisdiction

Telemedicine medical services are provided only in the states where Dr. Wacks is actively licensed.
Patients must physically be located in one of these licensed states at the time of each telemedicine visit.
Services are delivered in compliance with the applicable laws, regulations, and medical governing body requirements of the state in which the patient is located.

3. Nature of the Patient-Provider Relationship

By engaging in services, patients acknowledge and agree that:

A valid physician-patient relationship is established upon completion of intake, consent, and the initial consultation.
Medical recommendations will be made according to professional judgment, available clinical information, and the patient’s individual health history.
Patients are responsible for providing complete and accurate medical history, medication lists, allergies, and updates on health conditions.
Services may require coordination with other healthcare providers, laboratories, or imaging facilities when clinically indicated.

4. Insurance and Payments

Dr. Wacks does not participate in Medicare, Medicaid, or any private insurance programs.
All services are self-pay only and fees must be paid at the time of booking or as otherwise specified.
Patients are responsible for all out-of-pocket costs, including but not limited to laboratory testing, imaging, prescriptions, and referrals outside of this practice.
Superbills or claim forms will not be provided for insurance reimbursement.

5. Cancellations and Refunds

Appointments must be cancelled within 48 hours or a $50 cancellation fee will be applied.
No refunds provided for services rendered or coaching packages purchased.

6. Limitations of Telemedicine

Patients acknowledge and understand that:

Telemedicine has inherent limitations, including inability to perform certain physical exams or procedures.
If urgent or in-person evaluation is required, patients may be referred to a local healthcare provider or emergency department.
Telemedicine is not appropriate for medical emergencies. Patients experiencing a medical emergency should call 911 or go to the nearest emergency room immediately.

7. Privacy and Confidentiality

All medical services comply with the Health Insurance Portability and Accountability Act (HIPAA) to protect patient privacy and health information.
Secure telemedicine platforms are used to ensure confidentiality of communications and medical records.

8. Patient Responsibilities

By agreeing to these Terms & Conditions, patients commit to:

Providing accurate and complete health information
Complying with treatment plans, recommendations, and follow-up schedules
Communicating changes in health status promptly
Using telemedicine services only when physically located in a licensed state

9. Disclaimer

Services are provided within the standard of care but no guarantee of specific results can be made.
Information provided in consultations is not a substitute for emergency care, primary care, or specialty services outside the scope of peri-menopause and menopause management.
Dr. Wacks reserves the right to decline, modify, or discontinue services if patient safety, compliance, or professional boundaries are at risk.

10. Agreement

By scheduling and receiving services, patients acknowledge that they have read, understood, and agreed to these Terms & Conditions.
